If you would like to reproduce material from the Diagnostic and Statistical Manual of Mental Disorders (DSM), books, or journals (The American Journal of Psychiatry, FOCUS, Journal of Neuropsychiatry and Clinical Neurosciences, Psychiatric News, The American Journal of Psychotherapy, and Psychiatric Services) published by the American Psychiatric Association and APA Publishing, please complete sections I, II, III, and VI of our permission and license request form. GAF and SOFAS Scales
The APA no longer endorses or licenses the Global Assessment of Functioning Scale (GAF) or the Social and Occupational Functioning Assessment Scale (SOFAS) for clinical or patient care. The GAF Scale has been replaced by the WHODAS 2.0.
DSM-5® is a registered trademark, and all of its content is protected by copyright held by the American Psychiatric Association. All rights are reserved, and written permission is required from the American Psychiatric Association for use in any way, commercial or noncommercial.
Requests for permission to reproduce DSM-5® materials will be considered for these uses:

For all DSM-5® requests we require:
- Completed online permissions and licensing request form.
- A document attached to the online permissions request form that includes the tables as they will be reproduced in your publication. DSM material must match the source exactly, with the exception of style and format changes. - Uses needed, from the list above, must be clearly stated in the request. No blanket or derivative rights will be granted.
- Please note that for DSM-5® requests, we will only grant up to 50% of the total number of diagnostic criteria in the book.
Use of DSM-5® Content in Trainings/Seminars
Written permission is required from the APA for use of DSM-5® content in trainings/seminars, commercial or noncommercial.

For clinical trials, available as modifiable Microsoft Word files. The SCID-5-CT is available by permission only. Customers will need to procure permission per clinical trial, and the fee will be charged per number of copies reproduced. The file is available as modifiable Microsoft Word only. Customization of the file by Biometric Research is also available.
Quick Structured Clinical Interview for DSM-5® Disorders (QuickSCID-5)
A purchase of the print book includes a license to photocopy QuickSCID-5 interview components for private clinical assessment use, up to 200 total copies per customer without charge.

- Use of the DSM trademark, which includes all DSM editions, for any purpose
- American Psychiatric Association and APA Publishing logos
- Reproduction of entire DSM content in another publication or product
